Severe respiratory distress after having emotional argument

Question

A 19-year-old female presented in the emergency department in moderate-to-severe respiratory distress after having an emotional argument with her boyfriend. The patient was well known to the hospital staff. She had a long history of asthma and had been hospitalized for exacerbations of her asthma six different times over the past 3 years. The patient was 18 years old at the time of her last hospitalization. Upon entering the emergency department during her last hospital admission, she was unconscious and her arterial blood gas findings showed a pH 7.05, PaCO2 105, HCO3 29, and PaO2 41. She was immediately intubated and placed on a mechanical ventilator. She was extubated 3 days later.
