Nurses have several obligations related to advanced directives (AD's), based on The Joint Commission's standards and the ANA Code of Ethics.

Nurses must ask whether a patient has an AD on admission, and get something related to the answer into the current chart. If no AD exists, the patient has to be provided an opportunity to complete this paper-work by the nurse. Any questions the patient has about AD's, what they include or how they work, must be answered by the nurse. A copy of any existing AD must be obtained for the medical record, and an on-going assessment is conducted during the patient's stay to ensure their wishes do not change.
