Question

A 65-year-old man presents to you for a preoperative evaluation. He is scheduled for hip-replacement surgery next week for severe osteoarthritis. He is able to ambulate but is experiencing worsening pain that limits his activities. His medical history is significant for hyperlipidemia and gastroesophageal reflux disease. He takes fish oil and famotidine. Because of fatigue and hair loss, his thyroid-stimulating hormone (TSH) level was checked by the surgeon and was found to be 55 mU/L (normal range, 0.5-5 mU/L) with a free thyroxine (T4) level of 0.5 ng/dL (normal range, 0.8-2.4 ng/dL). His examination, complete blood count, basic metabolic panel, prothrombin /partial thromboplastin times, and electrocardiographic results are all normal. The surgeon would like to know your recommendations for the perioperative management of the patient's hypothyroidism. What should you advise?
