Question

A 14-year-old male presents with a right shoulder pain for 4 months not associated with trauma. The pain has become increasingly worse and is there all day and keeps the patient awake at night, interfering with sleep. An x-ray reveals a pathologic fracture. An initial complete blood count revealed a hemoglobin of 12 g/dL, platelets of 140,000, and a white blood cell count of 12,000. A biopsy of the site reveals an aggressive infiltrative process of the humerus, scapula and clavicle consistent with Burkitt's lymphoma. A PET scan reveals lesions above and below the diaphragm only.
