Scenario
You are a registered nurse working on the rehabilitation ward of a regional hospital. Over the past three months, you have identified ten patients with skin and tissue injuries that are localised to either their sacrum, greater trochanter or heel. All affected patients had limited mobility and were assessed upon admission as not having any skin integrity issues. You are given permission from the Director of Nursing to investigate the problem from a safety and quality perspective.
