Question

A 38-year-old female presents to your office for a routine physical and a flu shot. She offers no particular complaints other than fatigue, which she attributes to the demands of caring for her 1 year-old twin sons while also working full time. She denies any past medical or surgical history. Her physical exam is WNL, but excessive abdominal adiposity and her V/S and labs are as follows: V/S: 125/82 HR: 84 RR: 16 Temp: 98.4 HT: 5'5" WT: 227 lbs. Labs: all normal except fasting glucose: 135 and HgbA1c: 6.5%.
