Question

The patient is a 55-year-old male with known atherosclerosis of the lower extremities that was worse on the right lower extremity and causing multiple ulcerations of the right calf. For this reason, the patient was seen again in the wound care clinic. The physician examined the non-pressure ulcers on his right calf. The physician updated the patient's history while ­interviewing him and his wife, and included the fact that the patient has never had vascular surgery or a bypass procedure on his legs. The physician was pleased that the ulcers, that at one time were much deeper into the tissues, were now limited to the muscle without evidence of necrosis and were healing. The physician applied the wound healing dressings to the patient's right lower leg. The patient's wife was again instructed on how to apply the dressings at home. The patient was given an appointment to return to the wound care clinic in two weeks. The patient was known to have type 2 diabetes mellitus that can also cause the skin ulcers, and the physician reviewed the patient's blood sugar result taken during the visit. The patient has taken insulin on a long-term basis.
