He reports he has osteoathritis of right hip, right knee and on pain medication and uses a cane for ambulation. He was on pain management in VA and last he last took pain medications about a year ago.He stopped going there and started self medicating in the streets.Has glaucoma and uses eye drops on his right eye. He is partially blind on the left eye.He has hypertension.He denies surgical history.Denies medication allergies. He reports he was diagnosed with GAD six years ago, has sleep disorder, depression, bipolar and OCD. He reports he last took his anxiety medications this morning.He is not clear what he takes but he mentioned sertraline.
