PATIENT CASE: Subjective: MM is a 52 y/o female patient who presents to your clinic after recently being in the hospital for a fall. She is relatively confused and was quickly discharged from the hospital. She is concerned about her rheumatoid arthritis medications r which she says the hospital changed.

Medication List: Methotrexate 10mg every day Leflunomide 2 grams every day Atorvastatin 40 mg every day (to treat high cholesterol) Lisinopril 5 mg every day (to treat high blood pressure) ProAir HFA (albuterol) 90mcg one inhalation twice a day (to treat asthma) Cetirizine 10mg every day (to treat allergies) Omeprazole 50mg four times a day (to treat GERD)
