72 year old 148lbs 5 foot 4 inch female admitted to the MICU 30 minutes ago from a rapid response call for respiratory distress. While out on the general floor the pt's oxygen saturations dropped into the mid 70's on a 3LNC. She was complaining of SOB and cough that produced a moderate amount of pink frothy secretions. She continued to deteriorate requiring admission to the MICU on CPAP mask at 10cmh20 with 50% oxygen. She is confused, disoriented and repeatedly attempts to remove the CPAP mask. Assessment reveals: CXR - Bilaterally fluffy infitrates, Pleural effusions, Cardiomegaly and Pulmonary Vasular Congestion BNP - 1050 pg/ml Hypertensive - 150/106 Tachycardia - 110 Bpm B/S coarse crackles in the mid and lower lung fields bilaterally What disease state do you think this patient is presenting with and why? As you arrive in the patient's room you recognize that the patient distress on a CPAP mask and she continually pulls of mask. After reviewing the history and current condition of the patient what is your next course of action?
