Case Study: The Smith Family
Background: The Smith family consists of Michael (father, 38), Chodren (mother, 36), and two children from Chodren's 1st marriage Lola (daughter, 17) and Liam (son, 13) who has been diagnosed with Autistic Disorder. The family resides in a rural community where services for Liam are limited. Michael, a chiropractor owns his own business , while Chodren works full-time at a home for adults with various developmental disorders. The family's medical insurance is through Chodren's employment with Liam meeting criteria for government services. Chodren is from Uzbekistan where she was raised in a very well to do home under strict Muslim orthodoxy. She describes her parents as strict but loving with her father being the one that set the rules in the home. She married at age 16 but after 7 years of marriage she fled her first marriage due to significant domestic violence. She responded to Michael online ad through a site that specializes in finding spouses.
Michael was raised in the deep southern part of the US by a family that held strict evangelical Christian values. He describes his parents as having a strict but loving home with mom being the one that made the rules. Both report growing up in a home where the father worked outside of the home and the mother "took care of things at home." Michael has developed a successful professional practice with multiple offices throughout NY State. Neither of them professes any religious faith and do not follow the religious practices with which they were raised.
Presenting Problem: The Smith family has sought counseling due to escalating conflict and tension within the household. Chodren reports feeling overwhelmed and isolated, struggling with managing the household responsibilities and parenting the children. Michael acknowledges feeling disconnected from his family and experiencing increased stress at work. Both report that Liam's behaviors make it next to impossible to enjoy time together. Chodren responds to Liam's outbursts by providing him with a new toy which he breaks within hours. Michael stated "we have a shed full of broken and new toys.
When Liam breaks a toy she brings it to the shed and comes back with a new toy to keep Liam calm." Though they have a Medicaid Respite provider, Chodren refuses to utilize the service. Lola is withdrawn and exhibits signs of anxiety. She frequently takes Michael's car without permission and recently had a minor car accident with Michael's car. She is spending less time with the family and more time with her overbearing boyfriend. She has been missing school and may not graduate due to missing so much seat time. She previously helped with providing childcare for Liam but has become resentful towards "babysitting." Liam has been acting out at home and in school. He has a 1:1 aide in school.
This aide seems to be the only one who can manage Liam's behaviors and the family has hired her as their respite provider. Chodren complains that Michael wants to leave Liam with the respite provider when they go out to eat. Michael reports that you can't go in public without Liam having a violent outburst. Michael wants to go on vacation without the children and Chodren is resistive. The school has made a hotline call due to Liam coming to school with a bruise on his cheek and Lola's absenteeism. The therapist is recommending family therapy and wants to include the school aide/respite provider.
