Assignment:
I. Client Interview
For this part of the assessment, you are required to undertake an intake interview, on two (2) separate occasions, with two (2) clients. Arrange with your supervisor, access to elderly clients with complex care requirements.
Read all instructions carefully before proceeding.
Steps to take:
1. Review your organisation's policies and procedures for completing an intake interview for clients.
Guidance: These may have already been provided to you on your first day of vocational placement or during your induction and orientation in the service.
2. Request for your supervisor's authorisation to access the referral form or the details of the client. Review the client's details before proceeding with the intake interview.
3. Follow your organisation's policy and procedure for completing the intake interview. Ensure that you have:
a. Introduced yourself and the service and sought introduction from the client.
b. Engaged the client by establishing rapport and trust.
c. Identified the purpose of the interview, oriented the client, and sought the client's participation.
d. Ensured that the client is empowered and their independence is maintained.
e. Explored the most urgent issues/ risk or potential risk the client is experiencing and prioritised these with the client.
o Developed an action plan with the client that contains the following:
- Most urgent needs of the client (at least three)
- Desired result/goal for each need
- Activities/strategies that will be used to pursue each goal with a person-centred approach
- Responsible person/s who or service/s that will carry out each activity/strategy
- Timeframe for the activities/strategies
4. You may use your organisation's forms to document the intake interview, or you may locate the following templates below:

Guidance: To maintain the clients' privacy, the clients' will be referred to as Client A and Client B, throughout this assessment, and their names must be replaced accordingly in all documents submitted.

To complete the form, you are required to:
a. Provide specific descriptions of how you completed ALL tasks and activities.
b. Provide the date(s) of when you completed ALL tasks and activities.
c. Provide other details relevant to the completion of these tasks and activities, where required. For example, the clients you supported
while completing each task.
d. Have your observer sign his/her initials on the space provided for each task as they are completed.
1. Describe two (2) separate instances when you empowered an individual to identify and prioritise their own needs.
Guidance: Explain the strategies you have utilised to empower clients to identify and prioritise their own needs.
2. Describe two (2) separate instances when you provided an individual with service information and supported their decision-making process.
Guidance: Include in your explanation the specific service information you provided the client, as well as the steps you took to support his/her decision-making process.
3. Describe two (2) separate instances when you encouraged an individual to advocate on their own behalf to access services.
Guidance: Explain the strategies or the activities you used to encourage a client to advocate on his or her own behalf to access services that he or she needs.
4. Describe two (2) separate instances when you maintained and stored a person's information according to confidentiality requirements.
Guidance: Include in your description at least three (3) specific strategies/activities you employed during your vocational placement.
5. Describe two (2) separate instances when you sought feedback about assessment processes from the person and other networks.
Guidance: Explain when and how you seek feedback about the assessment processes from your clients and from other relevant stakeholders. Include the feedback provided by the client and other relevant stakeholders
6. Describe two (2) separate instances when you monitored processes and their outcomes in terms of success in meeting the person's needs.
Guidance: Explain the activities/strategies you employed to monitor processes and client outcomes in terms of meeting their needs.
7. Describe two (2) separate instances when you used feedback and own evaluation as a basis for improving processes.
Guidance: Specify a process that needs improvement, include feedback from at least one (1) relevant person you have worked with (e.g., client, supervisor, co-workers, etc.), provide your own evaluation, and how you used other's feedback and your own evaluation in improving a certain process used during your vocational placement.
8. Describe two (2) separate instances when you reflected on own performance.
Guidance: Reflect on your performance and answer the following questions:
? Do you think you were able to provide effective case management to your client?
? What aspects of the case management process could be improved? (Think about the process you used).
? What strategies (at least three) will you use to become a more effective and efficient manager?
9. Describe two (2) separate instances when you worked with the client to determine the services required.
Guidance: Include in your explanation the specific service requirements you have determined with your client.
10. Describe two (2) separate instances when you identified, implemented and maintained duty of care responsibilities.
Guidance: Explain how you provided the standard of care that is reasonable and consistent with duty of care responsibilities.
11. Describe two (2) separate instances when you obtained client feedback about services and used oral communication skills to maintain positive and respectful relationship.
Guidance: Explain the specific activities/strategies/steps you used to obtain client feedback on the services he/she received.
12. Describe two (2) separate instances when you identified and implemented further support required to meet changing needs and progress towards outcomes.
Guidance: Include in your explanation the following:

13. Describe two (2) separate instances when you applied a strengths-based approach to assessment.
