Scenario
As an International Board-Certified Lactation Consultant (IBCLC) and registered dietitian nutritionist (RDN), you have been doing consulting work for physicians specializing in obstetrics and gynecology. Most of your counseling has been geared to middle- and upper-class pregnant and lactating women. You have created nutrition education pamphlets outlining the benefits of breastfeeding and lactation management to promote continued breastfeeding. Most of your visits were to patients' homes. You also held group sessions focusing on nutrition and lactation for pregnant mothers enrolled in birthing preparation classes.
The community hospital in your area has set up "The Healthy Start Program" for pregnant women with incomes less than 185% of the poverty guidelines for prenatal and postpartum care and has requested your services in setting up a breastfeeding promotion program for program participants. The population is predominantly African American (70%), Hispanic (22%), and Caucasian (8%). You would like to start with initiating a Breastfeeding Peer Counselor (BFPC) program. Your budget includes sufficient funds to start a BFPC program, along with hiring three part-time staff members, who are IBCLC, to assist in educating and supporting pregnant and new mothers about breastfeeding.
1. Review the following Academy of Nutrition and Dietetics' (AND) latest position paper titled "Promoting and Supporting Breastfeeding," which is available from the Journal of the Academy of Nutrition and Dietetics. List the benefits of breastfeeding; choose one that you think is interesting and discuss. Then list the barriers to breastfeeding; choose one that you think is common and discuss.
2. What is the Healthy People 2020 goal for breastfeeding described in AND's position paper? How does this compare to the National Breastfeeding Rates listed for 2011?
3. Referring to your text under Breastfeeding Promotion, describe the rationale for using peer counselors to change culturally based behaviors.
4. In review of the breastfeeding trends in the United States, compare the Healthy Start population to the findings in the Academy of Nutrition and Dietetics' position paper.
5. Identify the barriers to breastfeeding that may exist in the Healthy Start population.
6. Compare and contrast your experience with counseling pregnant women on lactation in a physician's referral practice (you are the RDN in this case) to the present situation with low-income recipients in a Healthy Start program; take into account any bias you may have toward breastfeeding.
7. To make the breastfeeding peer counselor program a success, what strategies can you use to develop a breastfeeding peer counselor program? Search these websites and any other sources to find out more about peer counseling programs:
8. Judging on the basis of your interpretations in the questions posed in Step 2, what types of revisions would you make to your original nutrition education materials, counseling, and group sessions to help minimize barriers to successful breastfeeding in the Healthy Start setting? List websites that would provide appropriate educational materials on breastfeeding and other forms of resources such as social media.
9. What other resources can you use to help your staff become more proficient in promoting and supporting breastfeeding?
10. As you continue to work with breastfeeding promotion at Healthy Start, what steps would you take to encourage breastfeeding up to one year?
