Problem: Mr Jones is a 70 year old widowed who has been residing at sunnydale retirement home for the last year since his wife died. He has four sons who are supportive but live out of province. Mr Jones moved to the retirement home as his wife always managed the cooking, the cleaning, laundry and could not cope on his own. Mr Jones has adjusted well to the retirement home living but is upset that he cannot smoke in his room. he is often seen smoking under the covered patio when the weather is warm. He is clear about the fact that "smoking is my only pleasure". You have been assigned to care for Mr Jones today. You review his careplan and see he has advamced COPD and is on 2liters of oxygen via nasal prongs. You are responsible for assisting Mr Jones with his ADL's and walking with him to the dining room for his meals. Upon entering his room, you see Mr Jones with the nasal prongs placed above his nose and he is sleeping in his lazy boy chair next to his oxygen compressor. You gently wake him up and intoduce yourself. Within minutes you develop an instant rapport. As you are assisting him with hisADL's you notice he is very thin. He tells you, "I have no appetite". His face is pink and ruddy in color. His skin is warm and moist. Ypu check his temperature and his temerature is 38.1. He states, I feel hot, and his chest is rattling when he breathes, and he is coughing up thick white mucous. He tells you, "I have more sputum than normal the last couple of days and asks you to pass him a tissue. You notice the garbage can is overflowing with tissues.
