Task Summary:

In this assessment, you are required to establish and maintain two (2) quality systems for the projects assigned to you. Refer to the following instructions for competing the following projects.
Project 1: ATCWA Simulated work environment based
Resources required:

Project 1: Establish and maintain a quality system
(Simulated Work Environment Based)

This project is based on the case study below that stimulates an actual workplace situation and you will follow 6 stages to complete it. Read the case study below and complete the project 1.

Case Study: Sky-Line Quality Failure
Sky-line builders are one of the highly reputed, medium scale construction companies operating in Australia since its inception from 1998. Once of their recent construction projects that they undertook was a rooftop parking deck of a single story supermarket located in a busy shopping complex near the City Tower. The expected capacity of the rooftop deck was 500 parking bays for the gold class customers who will be paying an extra 20% of parking fees for their reserved parking bays.
The construction was initially commenced in April 2020 at the end of a 6-month project planning stage initiated in October 2019. The Sky-line builders assured to get all relevant stakeholders consulted in the planning stage including the supermarket management, key resources suppliers, engineers, draftsmen, technicians, quality assurance consultants. The construction of the deck was ended in December 2021; thus the grand opening was scheduled on the 01st of January 2022. However, the deck collapsed on the opening day, during its grand opening ceremony.
According to their chief site engineer "we never expected a disaster on the opening day... can't believe with the attention we paid to assure the quality of the materials and the procedures we followed... it's something that I don't understand"
The ceremony was a special preview opening for neighbourhood businesses and residents so they had been directed to park on the deck. The shopping complex included a hotel, apartments, retail space, theatres and some offices. After the welcoming program, the attendees began to shop and in total, there were more than 370 employees working in those retail and other stores. About 15 minutes later, a loud bang was heard, and water began to flow from a broken overhead fire sprinkler pipe. A photograph was taken of the broken pipe and a severely distorted beam-to-column connection. After this distortion and pipe rupture, the supermarket staff acted promptly and efficiently to clear people from the immediate area and then to begin evacuation of the entire store. Approximately 5 minutes later, the roof in four bays collapsed into the shopping area causing considerable building damage along with destroying 20 automobiles. No one was killed, but 27 people were injured.
An investigator was appointed two weeks after the failure by the City Council to investigate the failure. The investigator's report detailed the probable technical cause of the collapse and also reviewed the many procedural deficiencies that led to this failure. The recommendations included in the report continue to effect important revisions to standards of practice in Australia. The 8,400sqm supermarket building was a single story steel column and beam structure that used the cantilevered or Gerber beam system. Wide flange steel beams passed over hollow steel tube columns, extending as cantilevers at each end. The beams supported open-web steel joists, which in turn supported a composite concrete and corrugated metal deck. The most probable technical cause of the failure was insufficient stability of the beam-to-column connection. There was no provision in the design for lateral support to the bottom flange of the beam, at a condition of bending moment that placed this flange in compression. This type of failure has occurred in the past, and the need to investigate this mode of failure has been well-established.
Of greater interest than the technical cause of this failure are the many procedural deficiencies in the project delivery system that permitted the design deficiency to go unrecognized. The investigator's report indicated numerous contributing procedural problems, including not having set up standard procedures for maintaining a quality system, competitive bidding for design services, unclear assignment of responsibilities, inadequate involvement of designers during the construction phase, poorly-monitored changes during construction, incomplete peer review, and inadequate professional liability insurance. On a further note, the investigator was unable to gather relevant statistics and quantifying data as to there were no procedures related record keeping and monitoring of project activities. This situation hindered the extent that the investigator had to make a reasonable judgment on the project failure.

Project Stage 1:
Review and analyse the current system and practices to establish a new framework for managing quality
Assume that you have been appointed as the Quality Assurance Lead for the Sky-line Constructors. Your primary role is to review the failure of the rooftop deck project and based on the findings, establishing a new framework for the company's quality system to maintain and assure the quality of their future projects.

Task 1: Organisation Review (Situational Analysis)
Your review/analysis should include a methodology that explain how you:
Review current quality system and its practices for maintaining quality
Interpret Sky-line's work and quality requirements and expectations
Current issues and problems associated with maintaining quality
Staff, duties and their key responsibilities
Current policies and procedures (If accessible)
Statistics and reports applicable
Risk management plans (if provided)
Access documentation available to use for determining the efficiency of the current practices
Once completed, save your report as "Organisation Review/Situational Analysis" and submit along with other documentation for this assessment.

Task 2: Draft a quality establishment plan
Based on the situational analysis that you have conducted above, you are now required to develop a quality establishment plan that addresses the following.
Documentation and procedures that are required to establish and maintain a new quality system
Quality system responsibilities in job descriptions and duty statements
Procedures for sourcing resources and procuring them
Standards for quality management systems
work effectively with others to establish and maintain a quality system in a way that meets all required outcomes
Communication strategy that will be used to;
Implement an effective communication procedure and
equipment related to establish and maintain a quality system
Provide accessibility to quality system information to relevant personnel
Save your draft as "Draft Quality Establishment Plan" and submit along with other evidence required for this assessment.

Project Stage 2
Consultation Process
Task 3: Establish and maintain collaboration and consultation processes
At this stage, you have completed the following and are in a better position to share your initial work with the relevant stakeholders and consult them for establishing a new quality system.
Situation analysis/organisational review
Quality establishment plan
Based on these initial documentation, the next step requires you to develop a "Project Stakeholder Consultation Strategy" that covers the following.
Identify internal and external stakeholders to be consulted. Consider the failure factors of the parking deck project and ensure to consider all relevant stakeholders that you must consult.
Decide what methods you would use to consult them.
Identify what information to be collected and shared
Resolve issues raised according to workplace procedures
Methods you would use for communication outcomes


Task 4: Stakeholder Consultation
Once you have developed the consultation strategy, arrange a meeting with the following stakeholders of the Sky-line.
Meeting with the Sky-line General Manager (your Assessor) to present your drafts, seek feedback and further information related to:
The factors affecting quality of the company's projects
Lessons learned from the past
Barriers encountered during the parking deck project
Operational efficiency
Meeting with Site-Supervisors (Your colleagues)
Arrange a consultation meeting with your site-staff (Your trainer will arrange a group of your colleagues to attend this session). The purpose of this meeting is to;
Present the updated quality plan after consulting the General Manager. Prepare a power point presentation that includes the content of your quality system plan.
After presenting the plan, ask for their feedback and discuss the current practices in place for maintaining quality and;
Discuss with them the areas where improvement and or changes are needed.
At this meeting, demonstrate your competency in;
Presentation skills
Effective communication skills including listening
Negotiation skills and
Problem solving skills
Your Assessor will observe when you conduct this group meeting.
Submit the following to your Assessor.
Power point slides you used to present information
Minutes and or information collected during the consultation sessions

Task 5: Update Quality Establishment Plan
Based on the feedback and ideas gathered at the above consultation sessions, update your quality system plan and save it as "Updated Quality Establishment Plan".

Stage 3
identifying and assessing quality issues and
design procedures for treatment of quality issues
At the completion of stage 2 of the project, you have Identified existing and potential quality issues that Sky-line has experienced over the last few years. Strategic planning requires that a thorough analysis of the external environmental threats and trends that could adversely impact the organisation's operational flow and sustainability.
Thus at this stage, you are required to consider both internal and external factors to identify, assess and quality issues and design procedures for resolving those quality issues. Complete the following tasks.

Task 6: External Environmental Analysis
Conduct research on the following key aspects applicable to the infrastructure industry in Western Australia.
Legislation and regulatory framework
Standards
Code of practice
Current trends and threats
Statistics and records related to contemporary issues
Prepare a "Research Report" that includes your findings and submit it along with other evidence.

Task 7: Establish procedures
To complete this task, use your internal and external environmental analysis to establish procedures for identification of existing and potential quality issues that align Sky-line workplace procedures. The procedures you develop should cover the following key areas and support effective implementation of your quality system.
Where you require further consultation or information to develop below procedures, you may contact the General Manager or any other relevant staff (Your Assessor) via emails.
Monitor changes to the workplace to determine if additions or modifications to the quality system are required
Treat quality issues and trends to confirm compliance
Interim solutions to treat quality issues at their source are not practicable
Ongoing treatment of the quality system according to workplace procedures
Monitor quality activities throughout the organisation
Identify inadequacies
Provide resources to implement appropriate quality measures
Conduct training programs that meet quality system training requirements
Compile all procedures you have developed and label as "Quality Procedure Manual" and submit along with other evidence.

Stage 4
Implementation and evaluation of the quality system

This is the final stage of establishing a new quality system for Sky-line constructions. By now, you have developed policies and procedures that support implementation of the quality system.

Task 8: Implementation plan
Since an effective implementation plan that consists of appropriate monitoring strategies is key to success of any system, this task requires you to develop an implementation plan for the proposed quality system to monitor its success and make improvement. Your implementation plan should include the following.
An action plan that includes;
Key tasks to complete
Monitoring strategies for each task
Timeline for starting and finishing each task
Personnel who are responsible for each task
Key performance indicators to measure the effectiveness of the system
Cost associated with any task
Resources and equipment list including all physical and human resources needed.
Communication plan that you will use to communicate for implementing the system
Once completed, submit completed "Implementation Plan" along with other evidence.

Task 9: Evaluation and continuous improvement strategy
Having an implementation plan itself does not guarantee or assure the success of any system. Thus, evaluating the success of operational activities and practices through the implementation of monitoring strategies is significant. It helps organisations to the level of success and identify areas for improvement as well as to implement changes for continuous improvement.
Accordingly, this task requires you to prepare a strategy that explains how the proposed quality system will be evaluated and continuously improve towards minimising risks and threats. Remember, as to the case study, this is one of the key issues that was identified after the failure of the "Rooftop Deck Project". In developing the strategy, explain, how you would carry out the following tasks.
Evaluate the effectiveness of the system
Record the effectiveness of the system
Make recommendations for appropriate improvements
Evaluate compliance with legislation and code of practice
Relevant legislation, documentation, policies and procedures and confirm that the work activity is compliant
Communication techniques and aids to advise others of work activity and exclusion zones as well as to provide instructions
Once completed, submit the proposed "Evaluation and Continuous Improvement Strategy" along with other evidence for this assessment.

Task 10: Project Sign-off
After successful completion of tasks 1 - 9, you have come to the end of this project. If you experience any difficulty whilst attempting to complete any of above tasks, it is highly recommended that you consult your Assessor(s) as they have been provided with all required information and guidance to assist you.
To sign off the project, submit the following documentation to your Assessor on or before the due date of this assessment.
