As a new nurse in the local health department, you have been assigned to care for Mr. Robinson and have developed a therapeutic relationship in which he has gained a great deal of trust in you. Due to changes in funding for the health department, Mr. Robinson, like many individuals with long term mental illness, are waiting longer to be seen resulting in lapses in continued therapy and medication refills.
Mr. Robinson has been homeless and living on the streets since he was discharged from the state mental health facility months ago. He has not been able to have steady employment due to his chronic paranoid schizophrenia. His family has removed themselves from his life. He often skips medication doses and his symptoms exacerbate. Due to funding cuts, he cannot be seen more frequently at the health department, nor does he have access to telehealth. You are worried about his safety as he is becoming more paranoid.
Due to these concerns, you go to your nursing supervisor to seek assistance in getting additional help for Mr. Robinson. Your supervisor tells you that due to continued funding cuts for individuals with mental illness, the only way that you can help Mr. Robinson is to tell him to go to the Mini-Mart on the corner and wait for you to meet him at 1PM. Instead of meeting Mr. Robinson, she tells you to call the local police and tell them that the Mini-Mart is being robbed and give them Mr. Robinson's description. Your supervisor goes on to tell you that this is how she has been handling all of the mentally ill individuals who need additional services. She also tells you the local police are aware that this is how these individuals are being managed. Furthermore, she tells you while they cannot be seen in a timely manner in the local health department, mentally ill individuals receive some form of treatment in the penal system. They are provided with meals and a safe place to sleep while being managed with medications.
Still concerned, you must make a decision that is best for you and Mr. Robinson.
