Question

Carmella is a 79-year-old Italian lady recently admitted to the aged care facility for 3/52 respite care with the possibility of long-term admission. She doesn't speak much English and wears a hearing aid but she can understand basic communication if spoken to slowly and clearly, in simple sentences with slightly elevated tone. PMHx: Alzheimer's dementia (with recent increased confusion and forgetfulness), T2DM (on insulin BD), OA with chronic back pain, THR 2 years ago, constipation, incontinence, impaired mobility, a recent fall at home and recurrent UTIs. Initial assessment by the RN on admission and follow up in the first few days post admission have found Carmella to be alert but pleasantly confused to T,P,P. She ambulates with a 4WF but requires close supervision. Nursing staff have noticed that the skin around her sacral area is quite reddened and is at risk of breaking down. A simple padded dressing has been applied and is being reviewed every 2/7. She requires moderate assistance with showering, dressing and grooming. She requires set-up with meals.
