"Pt appears to be confused and very forgetful. Pt complained that meals served are not good and she is requiring extra calories because she is underweight. She said she had talked to dietary/kitchen on her needs. She brought with her the current menu which was not filled out. Writer called dietary/dietician manager and discussed her resident issues with both. Both confirmed that resident was there this afternoon discussing what she wants for dinner. Pt did not have any memory of having gone to the 5th floor and speaking with dietary/dietician. Writer helped resident fill out the weeks menu. A copy was sent to the kitchen, another given to the nurses to follow up if there is any complains. Nurses have reported pt. has ordered a lot of food which is stored in the fridge. Pt is informed that food is only stored for 7 days and then disposed off after the 7 days. "
