Client presents to the provider's office reporting increased generalized pain, states it is painful to take deep breaths.pain as 10 on a scale of 0 to 10. Client has not slept due to pain and is fatigued. The client has been taking the NSAIDS prescribed 2 days ago, the hydroxyurea as prescribed, and trying yoga without successful relief of pain. past medical history of sickle cell disease and takes 1000 mg Hydroxyurea daily PO. bowel sounds are hypoactive. Lung sounds are diminished throughout. Temperature 38.8° C (101.8° F) Heart rate 112/min Respiratory rate 22/min Blood pressure 144/92 mm Hg Oxygen saturation 95% on room air the client is at risk of developing dvt, pnenamonia, sickle cell crissi, or pulmory hyptention. as evidence by increase pain, fatigue, skin discoloration or chest pain.
