Client comes in today reporting fatigue and frequent headaches. Client reports always feeling tired, unable to sleep well, and wakes up early. Reports symptoms began approximately 5 weeks ago and are getting worse. Also reports a lack of interest in usual activities, missed work due to fatigue and an inability to concentrate, and frequent crying spells. Client states, "! don't care if I never wake up. I feel hopeless and worthless." Client is concered this may be "something bad." Loss of appetite with a 10 lb. weight loss in a month.
