Case study

A guy of 35 years old who suffers from a learning handicap and has a history of being abused and neglected. He has a history of engaging in self-inflicted violence as well as substance and alcohol misuse. He is presently jobless and residing at a facility that provides assisted housing. Anxiety, despair, and poor self-esteem are the difficulties he is now dealing with.

A lady in her 30s who suffers from a physical impairment and comes from a history of poverty and abusive relationships in the home. Because she has spent the most of her life in and out of hospitals, she has had very little opportunities to build social skills or participate in activities that have any significance to her. Her primary concerns consist of feelings of alienation, worry, and sadness.

Section 3: Reporting and evaluating processes

In this section, you are required to demonstrated how you report and evaluated processes about the disabled clients

1. Describe your workplace procedures for documenting the outcome of the assessment process. Attach evidence that you have documented the outcomes of the assessment process for this person.

2. Describe your workplace procedures for maintaining and storing client information, as well as in regards to confidentiality. Attach evidence that you have maintained and stored client information regarding confidentiality requirements.

3. Describe how you provided each person's information to other services according to consent and confidentiality requirements.

4. Describe feedback you received about the assessment processes use both from the person and others involved in the assessment process. You could also attach any specific evidence related to this such as completed surveys.

5. Describe how you monitored the assessment processes and outcomes to assess their success in terms of meeting the person's needs.

6. Explain the processes you used to seek feedback on your work. What feedback did you get? What does the feedback tell you about your performance in assessing the co-existing needs of each person?

7. Based on feedback and your own evaluation, describe at least one opportunity for improving processes.
