This is the first follow-up visit since last seen about 6 months ago, when he was referred for testing due to chest pain. He was unable to schedule any follow-up visits after hospital discharge, although he was advised to make an appointment with his primary care provider to discuss management of his coronary artery disease and his overall health, and to assist with decisions about how to proceed with care.
