Step 1 - Reflect on your current knowledge of the situation. For example, when you walk into a patient's room, you instantaneously and unconsciously reflect on your knowledge base. What do I already know about this patient that tunes me in to look for certain signs and symptoms?
Step 2 - What do I know from my experience that will help me now? For example, what do I know from my experiences with patients who have similar conditions?
Step 3 - What knowledge am I learning now that will help me in this situation? For example, what have I learned recently about this patient's condition and the regimens used to treat it?
Step 4 - What else do I need to know, and where will I find it? For example, reflections lead you to an important question to stimulate your critical thinking: What else do I need to know, and where will I find it?
Step 5 - Based on the first four steps, what should I do? For example, based on your reflections on your knowledge base, you may decide that you need to assess the level of consciousness or respiratory rate or examine the IV site.
Step 6 - Take action. For example, when you decide what assessments are most important, you are thinking critically. Your assessments combined with your knowledge will lead you to take action and implement specific interventions.
