The systems model of organizational accidents is also referred to as the swiss cheese model. Why do you think this nickname for the model came to be? Talk about a time a work, school, or in your personal life where this model could explain a problem (for example if you thought your first roommate was going to pay rent that month, that roommate thought that your other roommate was paying rent, and that third roommate thought you were covering it that month, the lack of communication between each of you, underlying assumptions, and absence of planning all combined to create this missing bill payment).
