Question

1st Visit Background: MK is a 36-year-old single male who is scheduled in your clinic today. You learn from his psychologist that he recently had a recurrence of psychotic symptoms and has been functioning poorly per his parents' report (he lives at home with his parents). MK has been receiving a prescription for risperidone 4 mg daily for the past several years, but his parents do not think he's been taking his medication because his behavior has deteriorated over the previous 2 weeks. MK had his first psychotic episode 8 years earlier when he was attending graduate school to obtain a PhD in biochemistry. At that time, he was placed on haloperidol (15 mg daily), which he took for 1 month and then discontinued on his own. His symptoms returned and he was rehospitalized. During this period, he dropped out of the PhD program and moved back into his parent's home. After multiple medication trials MK had been relatively stable on risperidone 4 mg daily for the past 2 years. Current symptoms include that MK believes other people can read his mind, and that they occasionally place thoughts in, or remove thoughts from his head. He is convinced that the.
