One initiative at my hospital is the implementation of a program called Meds to Beds that reconciles patient medications and provides medications at discharge, along with education given by a pharmacist. They use the Plan- Do- Study- Act to improve the process as the program progresses. There has been a steady rise in hospital readmissions within 30 days after discharge, which can sometimes be linked to a lack of medication understanding and resources.

They implemented this program to reduce readmission within 30 days of discharge and to promote antimicrobial adherence after discharge. The program aims to reduce the barriers that prohibit medication compliance. It provides medications before discharge and education to parents, teaching them proper and timely medication administration.

Translational research can influence this initiative because it gives a place to start without having to do studies ourselves. We can see what other hospitals have done regarding reducing readmission rates and medication adherence, and we can launch off from where they have already completed their research.
