Bradley and Taylor had many recommended approaches in tackling our healthcare paradox and the one that stood out to me the most was setting up a collaboration between healthcare providers and social services organizations. In the reading, Dr. Englander, as professor at OHSU talked about how at the time of discharging patients from the ER, the process is always difficult and unorganized. Patients don't have insurance, no place to live, no family support, no transportation, no money and so the transition from hospital to home is difficult. When patients are sent back to the community, they struggle with access to local resources like pharmacies, doctor offices, rides to appointments etc. Her and her colleague created a process called C-Train that get's all the right services and care providers for these services together to help patients and clinics smoothly transition patients into the community with the help insurance coverage, medication, facilitate housing and rehab services, to name a few. (p142). Bradley and Taylor believe that if we were to do this all across America, we could better support our patients and address the SDOH appropriately. A strength from this approach would be that the authors provide evidence from other countries.
