You must demonstrate the ability to assist with responses for at least two different workplace incidents. This might occur in a workplace or in a simulated workplace as organised by the assessor.

Identify 2 different workplace incidents, and address the bullet points below. Do not simply submit an incident report, although an incident report may be part of your submission.

Submit, to your assessor, a report outlining:

  • Each incident - type, severity, location, outcomes etc
  • The actions you took and the contributions you made with regard to the initial responses
  • The data collection processes you followed, after the incidents, to contribute to the investigation
  • How you contributed to compliance with legislative and organisational requirements
  • Any reports to which you contributed, including those for external authorities
  • Any recommendations resulting from the investigations
