Question: While Robert Sansone, vice president of energy engineering at Hartford Steam Boiler and Inspection Company, was visiting a fossil- fueled power plant control room for his company, an alarm horn went off. It's a very common occurrence in complex systems' control rooms. An operator reached over quickly and punched the override button to cancel the alarm. In the restored hush of the control room, Sansone asked the man what the alarm indicated. The operator asked what he was talking about. Sansone said he had just seen the man cancel an alarm by pushing a button. The operator denied it. "We went back and forth like that," Sansone told me, "until I checked the computer log and it showed that the operator had pressed the override button. Canceling alarms had become a rote."

Recommend either 1) a change to the display or 2) a change to process to fix this issue.
