Question

Remember Jack, the client who experienced increasing pain the at injection site after receiving his pneumonia vaccine by a medical student? He made a n appointment to see the doctor at the clinic and was found to have subacromial bursitis and a frozen shoulder. He discussed with the doctor his feeling that this pain was related to the pneumonia shot he received. He went on to file a claim against the clinic stating that the student was not adequately supervised. The Rest of the Story On expert evaluation, it was found that the vaccine had been administered into the joint instead of intramuscularly. The NP failed to supervise the medical student sufficiently and assess the competence of the injection technique. The patient's medical records were insufficient stating only that the injection was given in the left arm. There was no information on the batch number of the vaccine, and it was not clear who had administered the injection. The patient required steroid treatment and decompression surgery to treat the injury. This is an example of inappropriate delegation, and the NP remains accountable and responsible for the injury that resulted. Adapted from: Taylor, K. (2015). Principles of delegation and accountability. Practice.
