Assignment:

Two medics were on scene of a pediatric cardiac arrest. One paramedic asked "How much epi do we push?" The other, reading the pediat dosing tool, called out "0.4" "Copy. 0.4" and he pushed it. Problem was, one paramedic meant "0.4 mL" and the other thought "0.4 mg" With a concentration of 1:10,000, the patient ended up getting a 10x overdose. This situation may have been avoided by performing a Medication Administration Cross Check, or MACC. What are your thoughts on the MACC or a similar procedure? Have you used it before? Has it saved a potential med error? Do you have any other useful advice or ideas that might help prevent medication errors, especially at the end of a long shift when the paramedic has had little food or sleep? Should the MACC or similar check be required by protocol prior to administering a medication? Why or Why not?
