Question

The patient is a 62-year-old man who is 2 days post-op from a radical neck dissection and laryngectomy with a permanent tracheostomy for head and neck cancer. You are performing tracheostomy care now. When you remove the ties to change them, the patient coughs and both the inner and outer cannula pop out from the tracheostomy. What is your first action?
