Design a 10-question questionnaire based on your proposed research problem. The questionnaire must contain an introduction that explains the purpose, an approximate time frame for completion, and a statement on confidentiality/anonymity. There should be at most two open-ended questions. Please review the guidelines located in Chapter 15. You may also refer to Appendix 15A for wording on the most common research topics.
The questionnaire outlined below, consist of 10 multiple choice questions that is designed to develop a better understanding in the overall reduction in student attendance. The purpose of the questionnaire is to obtain information from students on their individual perspective to why they've opted to change their educational source. Additionally, the responses will be a resource for areas they may be in need of additional staff to fully meet the objectives of the college. Although the questionnaire is designed to be completed within a 10 minute time frame, we encourage everyone to take whatever time they to complete each question. The information you provide will be used to better your learning experience at the college. However, your answers and/or personal information will remain confidential. Additional comments are always welcomed at the bottom of the questionnaire; as this will provide an opportunity to include additional information or comments that may not be initially covered in the questionnaire.

The ultimate purpose of the questionnaire/study is to develop a student's perspective in the process of enrollment. Attendance records indicate that the triggers for the drop in enrollment is strongly influenced by the enrollment process.
To understand on a more in-depth level, questions, 1, 4, and 5 was directed at the overall attitudes of the staff encountered by the student during enrollment. Whether from Admissions, to Financial Aid, it is important to narrow down the areas that are in need of additional staff training. Ultimately, this step can also identify the need for changes in process or the rotation of staff.
Next, we want to understand the level of difficulty students are experiencing in obtaining the information they need to answer questions or determining the next step in the process. Questions 2, 3, 6, and 7 serves that purpose by gauging the staffs knowledge and whether the staff member is taking the time to meet the need of the enrolling student.
Question 9 gives an assessment on the student's mindset at the end of the process. Was in memorable in a positive or negative way. If negative, we want to make the experience seamless with the restructuring of process and the addition of technology for a smoother transition.
Finally, question 10 allows to student to include concerns they may be having that was not covered in the previous questions. The information provided for question 10 has the potential in determining the need for an additional survey to address these issues brought out through this question.
