Question

This elderly white female was presented to the hospital with a three day history of progressive cough productive of purulent sputum and mild shortness of breath. In the emergency room the patient was noted to be in atrial fibrillation, with a rapid ventricular rate, and there was concern that she may also have an early pneumonia. She was admitted and placed onintravenous and antibiotics and Cardizem drip. She was seen in consultation by Dr. Amil for control of her cardiac arrhythmia. The patient had significant difficulty with shortness of breath and cough, but was eventually able to tolerate a pulmonary regimen and was placed on Coumadin because of atrial fibrillation.
