The nurse is caring for Alice, a 45-year-old admitted to the psychiatric unit with a diagnosis of borderline personality disorder and an increase in violent tendencies towards self and others. This is a court mandated admission due to an incident at a public place. Alice states that she has been experiencing severe mood swings for the past month when her roommate left for an extended vacation. She is frustrated that her roommate has left her alone. Alice states that she is very angry about this and wants to hurt others so they can feel her pain. She also admits to self-harm through cutting and frequently walking the streets in order to obtain drugs. Alice admits to being promiscuous in order to obtain the drugs but that this is a new occurrence. Vital Signs: Baseline for client Labs: Toxicology report positive for alcohol, cannabis, and cocaine. Thyroid studies within parameters. Urine and blood HCG negative. Orders: Group therapy, individual therapy, mood stabilizer.
