You are a new psychiatric mental health nurse practitioner in a community-based, privately owned, for-profit practice. Today you have your first visit with a 41-year-old female who has been followed by another practice in the city for 3 years, but today she tells you that she does not like the new provider there, so she decided to start coming to you. She has a few copies of notes from her old provider that document diagnoses of (1) generalized anxiety disorder and (2) major depressive disorder, moderate, in remission. She says her depression appears to be in remission with a combination of citalopram 20 mg daily and bupropion XR 150 mg daily. She further reports that the only thing that helps her anxiety is alprazolam 1 mg that she takes twice a day. A PMP report reveals that she has been prescribed alprazolam 1 mg b.i.d. by a few different providers sporadically over the last few years. She refuses to sign a release so that you can see records from her last practice. Describe your specific plan of care for this patient's generalized anxiety disorder. What are the red flags in this scenario, and for what other things will you assess?
