Introduction
Now that you are familiar with the SHEL(L) model and its useful application in Crew Resource Management (CRM) discussion, you will have the opportunity to apply it to one of the most cited accidents in human factors literature. United Airlines Flight 232 was en-route from Denver's Stapleton International Airport to the Chicago O'Hare International Airport. However, over the skies of Midwestern green pastures and farmhouses something went terribly wrong. The #2 engine mounted on the vertical stabilizer experienced a catastrophic engine failure.

As shrapnel was being flung from the failing engine, it severed several hydraulic lines leaving the airplane crippled without flight controls.

Despite this horrific series of events, the flight crew lead by Captain Alfred Haynes, was able to land the aircraft at Sioux City, Iowa.

Though many perished in this unforgettable accident, the role of Crew Resource Management (at the time named Crew Leadership and Resource training at United Airlines) was instantly heralded by Haynes as one of the primary reasons anyone survived.

Instructions
Using the SHEL(L) model as your organizational tool, research the United Airlines Flight 232 accident. There are plenty of resources available regarding this well-known accident. However, some that may prove useful are:

Once you researched the United Airlines Flight 232 accident,  complete the SHEL(L) worksheet

. This is a simple exercise which helps you to identify several examples for each of the SHEL(L) model components within the United Airlines Flight 232 accident. For each component of the SHEL(L) model - S, H, E, and L - identify at minimum three components that exist for each within the United Airlines Flight 232 accident. You do not need to provide a paragraph response, these can be phrases or just enough to identify an example of the component. Write each example below the appropriate SHEL(L) model component. As mentioned, you must submit at minimum three examples for each component resulting in a total of 12 total answers.
