Question: Consider a mental health setting where you would like to work (such as a private practice, an outpatient mental health facility, a residential adolescent unit, a substance abuse treatment facility, etc.) and a population that you would like to work with even though you may still be figuring out your career goals for this assignment (e.g., child, adolescent, adult, substance abuse, college, career).

Make a consent document that includes information pertinent to your customer and the environment you selected based on your decisions. The informed consent should focus on the items listed in the Checklist for Informed Consent that is available at the link below and should incorporate aspects based on what we have studied in class and through the textbook.

The consent form should be between two and four pages long (not counting the lines for signatures) and written in plain language that the client can understand. Your selected environment and age group should be taken into account when developing the information (you can find samples on Corey and one in a link below). If you work with children, for instance, the informed consent should be drafted for the parents or guardians or you should provide the child a consent form that takes into account the involvement of the parents or guardians in the counseling process.
