Scenario - Moses
Moses is a 47 year old Aboriginal man who was in a fight when he was 39. Moses ended up with an Acquired Brain Injury and Paraplegia. Moses uses a wheel chair to get around but is unable to wheel his chair himself for any great distance. Moses is a large man weighing approximately110 kilograms and is over two metres tall.
Moses experiences short term memory loss due to his Acquired Brain Injury, and has good long term memory. He remembers a lot about his past and what his life was like before his accident. Moses receives a disability support pension. Moses also has type 2 diabetes.
Moses lives in a residential setting with six (6) other males who are all Aboriginal men from different communities in and close to the city. Moses has his own room with TV and a radio. He shares a bathroom with the other men. A spa bath was installed to assist Moses with his exercises. These have been put together by the physiotherapist to ensure he would maintain the muscle tone in his legs. Lately Moses has been saying that he wants to move into his own home.
Moses is married and has a son but has not had much contact with his son or wife since his accident. He did not want to go back to his community and will not say why. He doesn't have a strong connection with his community or his family since his accident.
Moses needs support with his personal hygiene, mobility and getting in and out of his wheel chair. He can eat his own meals that are cut up and placed on the table attached to his wheelchair.
His bedroom is equipped with ceiling hoists and mobile hoists. Moses is not really interested in assisting himself and he does expect that all his support including intimate personal supports is done by the support workers on shift.
Moses often does more for one support worker than another depending on what he thinks of them. He is very verbal and at times his language is inappropriate. He can be quite rude and insulting. Some support workers find it very difficult to provide support. Moses will say inappropriate things to see how you will react and that appears to shape the way he treats the support worker and shapes these relationship going forward.
Moses likes to watch pornographic movies and often asks the support worker on shift to go into town and get them for him. He likes to watch them at night in his room after he has gone to bed.
Moses likes to go shopping and he does like to catch up with people he knows in town. He prefers to attend functions held by the support agency but he tires easily and often needs to leave early. If this is not possible he starts swearing and hitting people as they walk past him.
He does not enjoy any physical activity. This includes his exercises developed for him by the physiotherapist. The exercises are to be done while Moses is taking a bath so that he has better range due to his body relaxing in the warm water. He also has three massages a week, one by a professional masseuse and the other by the workers that have been trained in this area. Moses skin integrity is very important and needs to be repositioned every two hours.
On Mose's 14th birthday, he was initiated into manhood and until his accident followed the traditions of his forefathers. He was also a respected man in his community.
Moses is on a variety of medications that are supplied by the pharmacist in Webster/blister packs on a weekly basis. The support worker's role is to ensure that the medication is checked and to assist with administration. The support worker needs to be aware that sometimes Moses holds them in his mouth and then spits them out at the support worker, or on the floor.
Moses does not really like spending his time with the other men living in the house as he thinks that they are (to quote) 'all wasted spaces' and he verbalises this. He often states 'I am not like them' they are all crazies'. This behaviour sometimes causes others in the home to react negatively towards Moses.
Moses uses an external continent aid (uridome) attached to a urine leg bag. This must be replaced daily after a shower and support workers need to ensure that the urine bag is emptied regularly throughout the day. Support workers must complete the fluid balance chart.
Overview of support:
- Support is 7 days a week 24 hours a day.
- Support shifts include:
- 3 workers on throughout the day 2 workers on night shift 1 worker sleepover
- Day shift two workers from 0700 till 1500 and the third worker has a split shift from 0700 till 0900 and then 1300 till 1900
- Night shift 1500 till 2200 one leaves and the other stays for sleep over
- Community Access workers when required.
