Both the text/ journal readings and the information on the website address in particular social economic factors on individual health and population health. They are called social determinants of health. They are circumstances in which people are born, grow up, live and age. In turn these circumstances are in part responsible for the unequal differences in health status within communities, and social ladder (CDC, 2013). In that aspect HealthyPeople.gov painted a grim reality of the impact of social determinants on wide range of health, functioning, and quality of life outcome. According to HealthyPeople.gov, certain social standings can make us more susceptible to poor health, shorter life expectancy, and poor health promoting behaviors. In turn, the Transtheoretical Model of Change reveals that social determinants are not a death sentence, but rather an opportunity to change. Prochaska et al., 2012 states that the stage of change model can help us understand how people in circumstances beyond their controls can attempt behavioral change that would ensure improved health and quality of life.

Now medical research, and the CDC are showing how measurable- and dangerous those social determinants can be. HealthyPeople.gov reveals new answers to why and how social determinants can be threat to the health of the individuals and the general US population. Healthypeople.gov reveals the fact about the impact of social determinants on wide range of functioning: Places where people live and eat affect their food choices, access to education, self-esteem, health promoting behaviors, smoking, use of illegal drugs and suicidal behavior. Crucial aspects worth mentioning that poor communities tend to have more liquor stores in every corner, fast food restaurants, worse education, worse work environment, and unsafe streets. This shows a strong correlation between community and health. The lower you are in the social ladder, the more likely you are to suffer from social determinants related diseases.

Traditionally research into social determinants has been associated with politics, government programs, and activisms. But here we look at  individual responsibilities, education, and behavior modification.
