Mr. Adrian Shani is a 41 yo presenting to ED at 0630hrs on Monday with progressive shortness of breath, scratching feeling in his throat, and diarrhoea. The day before while gardening at a friend's home, he tripped and accidently knocked over an old unused hive and was stung by several yellow jacket bees. He was taken to a nearby GP clinic where he was treated. Adrian had only generalised swelling with bumps from the stings and insisted he felt fine and would rest at home. His wife is an aged-care worker, she said she would care for him with cold packets and Panadol. She drove him to the ED. Stings - Bilateral upper limbs(x5), R and L hands (x8), R face (x2). Appears unwell, confused and wants to sit up, complaints of pain from stings 7/10, chest tightness, dizziness, nausea, itchiness, rash and bumps to face and arms, decreased urine output Past Medical History: bronchial asthma since childhood, hay fever, recently diagnosed with type 2 diabetes mellitus Regular medications - Metformin 500 mg once daily, Salbutamol (100 mcg/puff) PRN dose one or two puffs, repeated four-hourly, Cetirizine 10 mg tablets PRN once daily Vaccinations up to date for Tetanus and.
