Question

Client admitted late last evening around 2200. Report received from previous shift that the client required PRN nebulized albuterol treatments through the night. Client awake, sitting up in bed; alert and oriented to person, place, date, and situation. States did not sleep well due to having to "pee almost every hour, since midnight!" Reports dry mouth and thirst, and "not breathing as well right now as I was around midnight." Expiratory wheezes audible. Oxygen on at 2 L/min via nasal cannula. Heart rate rapid, regular. Bowel sounds present × 4 quadrants. Pulses present, capillary refill less than three seconds, no peripheral edema noted × 4 extremities. IV normal saline infusing at 75 mL/hour to left arm; site patent with no signs of infiltration.
