Question

Write a nursing note for a patient was is admitted to ARRH: 88 years old female was admitted from HCC with principal diagnosis of weakness, gait instability, low back pain, and frequent falls. She has a history of femur left femur fracture since 2023, chronic CHF, HTN, asthma, low severe stage glaucoma to the left eye, constipation with other comorbidity diagnosis. She is allergic to minocycline, alert, and oriented x 4. She uses commode for toileting, independent x1 with a rolling walker. Skin is intact and continent of both bowel and bladder control but had a loose stool today before she was discharged to ARRH. It's not clear if she is full code or not from the medical record. She is on a cardiac diet and takes her medication whole. She can make needs known.
