A primary health care nurse is required to provide health education and facilitate health promotion for illness prevention. Explore the target group of the primary health care service in your workplace and define health issues presented in your primary health care facility. It is important to understand that the health issues of people accessing primary health care services will vary depending on the community and target population. In relation to the commonly presenting health conditions in your work setting, choose an appropriate topic such as quitting smoking or alcohol or management of obesity for health education and health promotion.
Provide health education for ten-fifteen minutes to a community group where the identified problem is prevalent.
Summarise the target groups, common health issues presented in your primary health care facility and points you discussed in the health education and health promotion activity in the section provided below.
The Host Facilitator or EQUALS' Clinical Facilitator will observe your performance and complete the observation checklist (provided in the logbook) and assessment outcome sections based on the skills you demonstrated, and the documentation completed.
You should familiarise yourself with the observation checklist prior to undertaking the assessment.
