He has comorbidities of CAD, A-fib, systolic heart failure, HTN, Hyperkalemia, Hyponatremia, HLD. The Resident is here because he cannot perform his ADL function and control his blood sugar. She is DNR, communicate effectively with needs know. Alert and oriented x 3. Unsteady walking can walk with one person assistant and a walker. Lack ability to transfer safely, at risk of falling. She has a foley catheter to control urine retention, skin at both lower extremities at the top of the foot are scaling including the nails unkept including a skin tear at the left lower extremity.
