Question

Nathan is a 14-year-old male referred by his primary care clinician because of worsening anxiety. He describes his anxiety as "panic with people." Since early childhood, he has been uncomfortable in front of groups of people if asked to answer a question, make an announcement, or give a presentation. He becomes anxious when anticipating a social event. However, he has never received any mental health treatment prior to this visit. His parents label him as shy and anxious. He has a relatively small network of friends, though he desires more friends. He is afraid of being criticized or negatively evaluated. He did not join any sports or social groups in school for fear of experiencing "panic" in the group or embarrassment. He has not yet had any dating experience, and this is a source of embarrassment with his peers.
