KR is a 15-year-old female who presents to clinic today for her annual well-exam. KR is in 10th grade and gets "pretty good grades," but sometimes feels unmotivated, tired and irritable. She sleeps about 6-7 hours a per night and thinks this may explain why she sometimes feels like she doesn't have enough energy to do the things she wants. She previously loved playing softball but has decided not to play this season. KR states that she has a few good friends and gets along for the most part with her parents and siblings. Denies any previous mental health issues or thoughts of suicide. Reports some difficulty falling asleep. Denies mood swings, aggression, seizures, headaches or forgetfulness. MOC reports some concern for increased irritability for KR. PMH: No sig PMHx for the patient. MOC has a hx of depression and takes Lexapro daily. No other significant family, social hx. Sleeping: Sleeps for 6-7 hours per night. She struggles to fall asleep. Reports usually "thinking about a lot of things before bed." She typically watches TikTok for several hours before sleeping. Appetite: Family eats a varied diet. Pt likes a variety of fruit and vegetables. She tends to skip breakfast most.
