Darius De Luca, a 61-year-old male client, presents to the clinic complaining of straining to urinate, a weak urine stream, difficulty emptying the bladder, and dribbling at the end of urination for the past six months. The NP administers the IPSS with results demonstrating severe symptoms. The NP performs a digital rectal exam which demonstrates a smooth but enlarged prostate on palpation. The rest of the physical examination is unremarkable. The NP concludes the visit and orders a urinalysis, urine cytology, and multiple labs including a PSA blood test for the client to complete. When the results return, there are no signs of infection or cancer. The NP diagnoses the client with BPH and considers what medications are appropriate for the client.
