Question

Patient presented as calm and cooperative, alert and oriented x3. Pt presented with increased symptoms of feeling down, depressed and feeling hopeless at times with a low motivation and decreased interest in doing most things. Patient expressed having symptoms severely feeling tired and having no energy to do anything .Pt. reports having fair appetite and reported having emotional feelings worried about the current state of functioning.Pt also reported having increased feeling of nervousness and anxiety, has been worrying continuously and has had increased trouble relaxing. Pt also reported being restless with poor concentration. During assessment ,pt. expressed with fair affect, engaged and responded to assessment questions.Patient denied having any substance abuse. Patient was recommended medication.patient was educated about benefits and the risk of those medications and was agreeable to starting the regimen.
