Overview:

For this homework, you will be writing a case note and referral summary based on the case of Greg. After watching the Chapter 6 Helper Studio video in Mindtap, you will assume the role of an intake interviewer for 211, a resource referral source, and draft a case note using the SOAP note format based on your meeting with Greg.

Once your case note is completed, you will prepare a referral summary for one service provider. This referral summary should be brief and should also follow the guidelines of confidentiality. When preparing your summary, determine what information is necessary to share in order to make the referral.

Instructions:

A. Create your case note based on the information Greg shared in the Chapter 6 Helper Studio video in Mindtap.

1. Your case note should be broken into the following sections to address each aspect of the note:

a. Subjective Data
b. Objective Data
c. Assessment
d. Plan

2. See Table 6.1 on page 200 of your textbook for an example.

3. Refer to www.211.org as a reference.

B. After writing the case note, prepare your referral summary for one service you think would be beneficial to Greg (i.e. individual counseling, parenting, job training, substance abuse education/support services, etc.).

1. Remember to remain objective in your referral summary.
2. Be sure to include the following information in your summary:
a. Client name and demographic information
b. Reason for referral/presenting problem
c. History of the problem if known
d. Client's relevant personal history.
